Concordia Plan Services is proud to offer the Concordia Disability and Survivor Plan (CDSP) to eligible workers. All full-time workers (those who are employed more than 20 hours per week and more than 5 consecutive months a year) must be offered the opportunity to enroll themselves and any eligible dependents in the CDSP. Part-time and temporary workers are not eligible for the CDSP.
The CDSP provides a disability benefit (for the worker) and a pre-retirement lump-sum death benefit (for the worker and enrolled dependents).
The CDSP pays a monthly income benefit equal to 70% of monthly compensation when a worker becomes disabled due to a qualifying disability (subject to the terms and conditions of the Plan).
Workers should call Concordia Plan Services and ask to speak to a Disability Benefit Specialist to report a disability that will result in missing 14 or more days of work. Claims need to be made as soon as reasonably possible (or when the worker can report the last day worked), and no later than 90 days after the disability begins.
The amount of disability benefit that a worker is eligible to receive is calculated based on his or her current salary.* If a worker was enrolled in the CDSP on or after January 1, 2005, he or she may be subject to the Pre-Existing Condition Limitation. * The disability benefit paid by Concordia Plan Services may be reduced by state mandated benefits or other forms of disability income/benefits such as Social Security disability benefits.
Effective July 1, 2007, the pre-retirement, lump-sum death benefit payment is
equal to two times a worker’s annual salary as reported by their
employer on the annual compensation report. An additional, one times
annual salary is paid for each eligible dependent child (up to four
children), for a maximum potential death benefit equal to 6 times a
worker’s annual salary. In the case of the death of an enrolled
dependent, a lump-sum benefit of $10,000 will be paid.
